This award was established to recognize a long-serving member who has made an ongoing contribution to the success of CAPhO. The award is available annually and consists of a certificate and cash award of $1,500.
The candidate must be a CAPhO member who has paid their annual dues prior to February 1st of the nomination year. They must have been an active member of CAPhO for a minimum of 5 years, having served on at least one committee or taskforce for CAPhO or held 1 term in office on the Executive Committee. The candidate must demonstrate leadership within the realm of oncology pharmacy practice and at the CAPhO Conference. Please note that members can only receive this award once.
Call for nominations
The call for nominations will be posted on the CAPhO website in January of each year for nominations for the previous years' award. It will be also included in the CAPhO E-newsletter and in social media posts. The deadline for nomination will be announced at the same time.
How to nominate
The person nominating should submit a 1-2 page letter indicating why the nominee would be a good candidate for the award and detail all their relevant contributions. This letter should include all contact details of both the nominator and the nominee and submitted via the online Awards and Travel Grants Application site.
Selection of candidates
Once the deadline for nomination has passed, all of the nominations are reviewed by the Awards Committee and the successful candidate is chosen based on the criteria in this description as well as the content of the application letter.
