Preceptor / Mentorship Award

This award was established to recognize a CAPhO member who has promoted oncology pharmacy through mentorship or preceptorship and/or who has sought student engagement within the profession. The award is available annually and consists of a $500 cash reward, certificate and a CAPhO Conference travel grant up to a maximum of $2000.

The Travel grant will be forfeited if not used the year it is awarded.

Eligible candidates

The candidate must have been an active member of CAPhO for a minimum of 3 years and must have paid their annual dues prior to February 1st of the nomination year. The candidate must work in a recognized institution of learning or place of employment and must have taught, engaged, or mentored at least one student in the calendar year of nomination. Members can only receive this award once. 

Call for nominations 

The call for nominations will be posted on the CAPhO website in January of each year for nominations of the previous years' award. The call will also be included in the CAPhO E-newsletter, sent as an announcement to members, and in social media posts. The deadline for nomination will be announced at the same time. 

How to nominate

Nomination is not limited to individuals with a student relationship to the nominee. Any CAPhO member is invited to nominate a colleague who demonstrates the spirit of this award. The person nominating is required to submit a 1-2 page letter indicating why the nominee is a good candidate for the award including details on all the relevant contributions of the nominee and a few vignettes of the nominees engagement. The letter must include all contact details of both the nominator and the nominee and be submitted via the online Awards and Travel Grants Application site.

Selection of candidates

Once the deadline for nomination has passed, all of the applications are reviewed by the Awards Committee and the successful candidate chosen based on the criteria in this description as well as the content of the application letter. The Awards Committee reserves the right to forego awarding this award.

Presentation of award

The award recipient will be announced and presented at the Annual General Meeting in the same year as the nomination.

Eligible expenses 

Eligible expenses are conference registration, hotel accommodations at the negotiated symposium rate plus taxes to a maximum of 3 nights, economy airfare and ground transportation. 

Responsibilities of recipient  

While attending the CAPhO Conference the recipient is invited to complete a short shift at the CAPhO booth in order to promote the CAPhO Awards and Grants Program. 

Reimbursement 

The recipient will be reimbursed based on original receipts to a maximum of $2,000. If less than $2,000 is spent, the recipient will only receive their total expenses.
