Fellow of the Canadian Association of Pharmacy in Oncology (FCAPhO) Recognition Program

The FCAPhO recognizes excellence in the practice of oncology pharmacy in Canada by distinguishing oncology pharmacists and other health care professionals interested in oncology pharmacy who are CAPhO members and have made outstanding contributions to this field and to CAPhO. There are three (3) awards available each year which consist of a certificate, Space pen, use of the FCAPhO logo, and a CAPhO Conference Travel Grant up to the value of $2,000.

The Travel Grant will be forfeited if not used the year it is awarded.

Eligible Members

In order to be successful, you need to document that you meet the minimum eligibility requirements, which includes that you are a practicing pharmacist for at least 10 years, a current CAPhO member, and have been a CAPhO member for at least 5 years, demonstrate that your professional activities exceed routine job functions, and illustrate how you have:

  1. Made a substantial contribution to CAPhO,
  2. Made practice contributions to pharmacy in the fields of oncology, hematology, and blood and marrow transplant,
  3. Participated in original research, and
  4. Participated in volunteer service, philanthropic or advocacy activities related to oncology pharmacy practice.

How to Apply

As a CAPhO member, you may self-nominate and submit an application package via the online Awards and Travel Grants Submission Site containing the following components:

  1. Completed online application form which includes a brief candidate statement and a request to upload your Curriculum Vitae (CV) and your photo
  2. Nomination letters (see below for details)

The call for applications is now closed and will reopen in the Fall.

Nomination Letter information

To become of an FCAPhO recipient, a member must be nominated by two current CAPhO members. The nominators must meet the following criteria:

  • Must be a current CAPhO member in good standing (Active, Supporting or Student (as long as he/she is not a current student of the nominee))
  • Must NOT be a current member of the FCAPhO Task Force
  • Must know the nominee well and feel confident in nominating him/her for the FCAPhO
  • Must be able to submit the nomination letter by the application deadline

Please forward the Nomination Letter Information to your two nominators, underlining the deadline to ensure your application is complete. Details on where to send the letter are in the Nomination Letter Information document.

Selection of Candidates

Your application will be reviewed by the FCAPhO Task Force which will make a recommendation of FCAPhO to the Board of Directors. The FCAPhO Task Force will base its recommendation on the information provided during the application process.

Presentation of FCAPhO

You will be notified of the status of your application in advance of the CAPhO Conference / Annual General Meeting, where FCAPhO recipients will be announced. 

Use of FCAPhO Logo

FCAPhO recipients are permitted to use the FCAPhO logo in email signatures, CV’s, applications and other communications. Details on how to download the logo files and style guide are provided to you upon notification of your FCAPhO status. 

CAPhO Conference Travel Grant

As part of the award, you are entitled to a Travel Grant of up to $2,000 to attend CAPhO Conference in the year your FCAPhO designation is announced. The Travel Grant will be forfeited if not used that year.

Eligible Expenses 

The eligible expenses are:  CAPhO Conference 2019 registration fees, economy airfare, accommodation at the conference hotel (at the negotiated conference rate including taxes and to a maximum of 3 nights) and ground transportation (up to $150).

Responsibilities of Recipient  

While attending the CAPhO Conference you are invited to complete a short shift at the CAPhO booth in order to promote the FCAPhO designation.  

Reimbursement 

You will be reimbursed based on original receipts to a maximum of $2,000. If less than $2,000 is spent, you will only receive the total amount spent.
