Larry Broadfield Distinguished Service Award

This award was established to recognize a long-serving member who has made an ongoing contribution to the success of CAPhO. The award is available annually and consists of a $2000 Travel Grant, certificate and cash award of $1,500.

The Travel grant will be forfeited if not used the year it is awarded.

Eligible candidates  

The candidate must be a CAPhO member who has paid their annual dues prior to February 1st of the nomination year. They must have been an active member of CAPhO for a minimum of 5 years, having served on at least one committee or taskforce for CAPhO or held 1 term in office on the Executive Committee. The candidate must demonstrate leadership within the realm of oncology pharmacy practice and at the CAPhO Conference. Please note that members can only receive this award once. 

Call for nominations

The call for nominations will be posted on the CAPhO website at the end of each year for nominations for contributions during that year. There will be an announcement in the CAPhO E-newsletter and as well as a notification sent to all CAPhO Committee and CAPhO Taskforce members. The deadline for nominations was Monday, December 10, 2018.

How to nominate

The person nominating should submit a 1-2 page letter indicating why the nominee would be a good candidate for the award and detail all their relevant contributions. This letter should include all contact details of both the nominator and the nominee and submitted via the online Awards and Travel Grants Application site.

Selection of candidates  

Once the deadline for nomination has passed, all of the nominations will be reviewed by the Awards Committee and the successful candidate will be chosen based on the criteria in this description as well as the content of the application letter. The Awards Committee reserves the right to forego awarding this award.

Presentation of award

The award recipient will be announced and presented at the CAPhO Conference Social Evening or Annual General Meeting in the same year as the nomination. The winner is invited to attend the following years' CAPhO Conference with all eligible expenses paid.

Eligible expenses 

Eligible expenses are conference registration, hotel accommodations at the negotiated symposium rate plus taxes to a maximum of 3 nights, economy airfare and ground transportation. 

Responsibilities of recipient  

While attending the CAPhO Conference the recipient is invited to complete a short shift at the CAPhO booth in order to promote the CAPhO Awards and Grants Program. 

Reimbursement 

The recipient will be reimbursed based on original receipts to a maximum of $2,000. If less than $2,000 is spent, the recipient will only receive their total expenses.
