CAPhO members volunteer many hours to the Association as part of our efforts to improve oncology pharmacy practice. Each year one member volunteer will be recognized for their contribution with a CAPhO Volunteer Recognition Award. Open to both oncology pharmacists and pharmacy technicians/assistants, this award consists of a certificate, registration to the CAPhO Conference, accommodation at the conference hotel and travel expenses of up to $2,000.
Candidates must be CAPhO members who have paid their annual dues prior to February 1st of the nomination year and volunteered in some capacity for the Association.
Call for nominations
The call for nominations will be posted on the CAPhO website in February and will be for nominations for volunteer contributions during the previous years. There will be an announcement in the CAPhO E-newsletter and as well as a notification sent to all CAPhO Committee and CAPhO Taskforce members. The deadline for nominations will be announced at the same time.
How to nominate
The person nominating should submit a one-page letter indicating why the nominee would be a good candidate for the award. The letter should outline the member’s volunteer contributions and any other relevant information. It should also include contact details for both the nominator and the nominee and submitted via the online Awards and Travel Grants Application site.
Selection of candidates
Once the deadline for nomination has passed, all of the nominations will be reviewed by the Awards Committee and the successful candidate will be chosen based on the criteria in this description as well as the content of the application letter.
Presentation of award
The award recipient will be announced and presented at the CAPhO Conference Social Evening or Annual General Meeting in the same year as the nomination. The winner is invited to attend the following years' CAPhO Conference with all eligible expenses paid.
Eligible expenses are conference registration, hotel accommodations at the negotiated symposium rate plus taxes to a maximum of 3 nights, economy airfare, ground transportation and meals to a maximum of $50/day for 4 days.
Responsibilities of recipient
While attending the CAPhO Conference the recipient is invited to complete a short shift at the CAPhO booth in order to promote the CAPhO Awards and Grants Program.
The recipient will be reimbursed based on original receipts to a maximum of $2,000. If less than $2,000 is spent, the recipient will only receive their total expenses.
