We invite you to join the Canadian
Association of Pharmacy in Oncology (CAPhO). CAPhO is a voluntary organization
for pharmacists and other health care professionals interested in the practice
of oncology pharmacy in Canada. CAPhO represents the professional interests and
issues of oncology pharmacy at a national level. CAPhO will offer services,
publications and discounted registration rates / travel sponsorship eligibility
for future NOPS meetings (should there be a registration or travel sponsorship
in future).
This is an application form for membership year 2008.
